Overview
This how-to article will help TeamDynamix Application Administrators create forms using TDAdmin. This article will cover ticket types, form configurations and next steps to assign a form to a service within the public facing client portal.
To complete the following steps, TDAdmin privileges are required. If you already have TDAdmin access, here's how to navigate to it: open the TDNext ticketing application, then navigate to the top-right corner and click on the settings wheel. From the dropdown menu, select "Admin" to access the TDAdmin panel

Ticketing Application – Ticket Types
Ticket type determines SLAs, reviewers, time and expense accounts, and are also used for reporting.
To create a ticket type category:
- In TDAdmin, navigate to Shared Ticketing Settings > Type Categories.
- Click +New.
- Name your type category
- Give it an order of how you want it to appear in the ticketing application
- Click Active
- Click Save
To create a ticket type:
- In TDAdmin, navigate to Types.
- Click +New.
- Choose a category
- Name your ticket type and describe if you would like to
- Add a Primary Reviewer
- Mark Active
- Save
Creating Forms
Forms in TDAdmin define routing, attributes, and classification. Follow these steps to create a form efficiently:

Access Forms
In TDAdmin, go to the Forms section.

Create a New Form
Click +New to start creating a form.
Tip: Instead of building forms from scratch, we recommend copying an existing form to save time and ensure consistency.

Form Details
- Name the Form: Give your form a clear, descriptive name.
- Choose a Classification: Select the appropriate category for your form.
- Copy from Existing: If applicable, copy attributes from an existing form for efficiency.

Customize Attributes
Add or remove attributes as needed to meet the form’s purpose.

Save the Form
Once satisfied with your form, click Save.

Preview the Form
Use the Preview tab to view how your form will appear in both the Client Portal and TDNext, ensuring it looks and functions as expected before deploying.
Creating Custom Attributes
Custom attributes are highly configurable fields that allow you to capture tailored data specific to your department’s requirements. These attributes can streamline data collection by enabling multiple-choice questions, checkboxes, or text inputs. Here's how to create custom attributes:

Access Attributes
In TDAdmin, navigate to > Attributes.

Create a New Attribute
Click +New to define a new custom attribute.

Configure the Attribute
- Name the Attribute: Assign a descriptive name that makes it easily identifiable.
- Header Text: Provide text for the header, which will display in reports and help identify the attribute's purpose.
- Attribute Type: Choose the type of input from the following options:
- Checkbox List
- Yes/No Dropdown
- Text Box
- Radio Button
- Number Field
- Date Picker
- Multi-select List
- File Upload
- Sort Order: Determine the order in which the attribute appears on the form.

Define Options
If the attribute type requires predefined options (e.g., for a dropdown or checkbox list), fill in the appropriate choices.

Save the Attribute
Once all configurations are complete, click Save to finalize the custom attribute.

To link your newly created form to a service in the Client Portal Application, please
submit a ticket to I.T. Please detail what service your form belongs to or if a new service needs to be created.
